- Runner
- BMI 23
- 2 years of knee pain
- Goal–Return to running
Patient’s Knee History
- Exercise, stairs, bending caused pain
- Recurrent injury over 2 years
- Pain frequency 8
- Prior treatment includes injections and physical therapy
Diagnosis
- Arthroscopy revealed 2.5 x 2.5 cm ICRS
- Grade 2 patella lesion inferior pole
Surgical Approach
- Lateral subvastus
- Evert patella
- Template: 2.5cm2 patella
- 6-0 Vicryl stabilize graft
- Micro-anchors inferiorly (reloaded with 5-0 Vicryl suture)
Rehab
- POD # 3
- 3/10 at 1st visit at 2 weeks post-op
- CPM -10-20°increasing slowly 6-8 hours 4 week with goal Flexion 120° by 6 weeks
- TTWB - 25% WB at 2 weeks, 50% WB at 2-4 weeks and then full WBAT 4-6 weeks
- Immobilizer locked in extension with gait
- 8-12 weeks:
- CORE program
- Limited open-chain exercises at 3 months and full allowed after 4 months
